Essential Functions
- Conducts appropriate on-site patient intake assessments in a timely and thorough manner utilizing validated screening and diagnostic tools.
- Develops appropriate plan of care for each patient admitted and updates as necessary in a timely manner.
- Conducts ongoing risk assessments on each patient, updating patient’s care plan when necessary.
- Assesses patient’s learning needs, identifies learning style and barriers to learning and assesses the patient’s readiness to learn.
- Conducts ongoing behavioral health counseling (individual and /or group) for each patient admitted, demonstrating medical necessity in ongoing documentation.
- Provides referrals for outpatient testing and therapy when appropriate.
- Identifies patient’s preferred learning style and barriers to learning and assesses each patient’s readiness to learn.
- Identifies situations when including family and other outside contacts into patient’s treatment is indicated.
- Collaborates with other school and healthcare professionals as required, including Nurse Manager and Sr. Mental Health Therapist.
- Works with school staff to develop classroom sessions and staff development aimed at enhancing knowledge of behavioral health issues across the school community.
- Educates school staff on signs and symptoms of behavioral health disturbance including impact of trauma and other risk factors and availability of school-based resources.
- Works with classroom staff to develop concrete classroom behavior management strategies to support students in self-regulation efforts.
- Maintains appropriate behavioral health information in each patient’s medical record to comply with insurance and regulatory reporting requirements.
- Secures and updates appropriate insurance authorizations to allow therapist to conduct patient’s treatment.
- Continuously monitors medical necessity for treatment and discharges patient in a timely fashion when appropriate.
- Maintains communication with patient families and guardians and provides appropriate information on case management, and referrals, if necessary, upon discharge.
- Makes him/herself available to assist school staff to plan for, evaluate and manage emergency and crisis situations.
- Maintains appropriate licensure and secures ongoing professional education to enhance knowledge and performance.
